Professional Personal Care Services (California Certified Personal Care Provider)
We provide In-Home Consultations for both, personal care and in-home health care support. We construct a care plan based on observation, personal Physician, and Family information. We then present the plan to the family for signing. We attempt to closely match Home Health Aides, and professional caregivers, homemakers, professional workplace attendants to the individual requiring assistance They are matched based on the skill level required, personality, and availability. The provider is familiarized with the required care needed and given details of any special needs. After a provider is placed we follow up with monitoring and weekly review. Examples of Personal Care Services are as follows: Personal hygiene, incontinence care, companion care, light housekeeping, outings, and visitation assistance, daily walks, plant care, and gardening assistance, etc..

In-Home Health Care Support and Facility Care Provider Guidelines
(Federal & California Certified Health Care Support Provider)
We provide non-medical (we do not replace a visiting nurse or physician service) in-home health care support services for seniors and the disabled. We are dedicated to providing the assistance necessary to remain in a home or independent living environment. Our team of highly experienced and dedicated staff is made up of certified nursing assistants, home health aides, and caregivers who are qualified to perform a variety of healthcare support obligations.
Health Care Support Providers:
Once an individual is released from a hospital or rehabilitation facility, they are given aftercare instructions. The instructions are to be performed on a daily or regular basis at home by the individual, a care provider, or a family member. We follow the aftercare instructions. The care instructions as listed are very important to the short-term, long-term, or permanent improved health, well-being, and or comfort of the individual. Our Health Care Support Services include; Alzheimer care - Dementia care - Parkinson's Disease - Hypertension care - Aftercare for hospital or rehabilitation stay - home hospice support - post-surgery wound care - diabetes home care - quadriplegic care support - dietetic meal preparation - medication management - charting vital signs, etc.. We assist family member caregivers by providing respite care, vacation care, or extended care services. We provide free caregiver support groups. We also provide free advocacy services to seniors and the disabled.
Price Quote| Sliding Fee Scale Per Month Once, the personalized care to be provided is determined and agreed upon, we will present a detailed price quote. Hours required per day, the number of days required per week or month is adjusted, as needed to fit the schedule of the loved one or the care client.
Contract and Signing| We will present a contract for signing before care services are scheduled to begin. All contracts are complete with a detailed, itemized list of care services to be performed, price, and length of the contract term. The length of a contract can be extended at any time.
Monthly Contract Rates| Available upon request. Monthly rates are more cost-efficient than weekly rates. Most monthly rates are limited to five days per week. One weekend day, per week, maybe included within each week of the month at no additional cost. Example: (Tuesday through Saturday with Sunday and Monday off)Contract Termination| A contract may be terminated by either party, with good cause, or by mutual agreement. Advance payments will be returned. There may be a 15% termination fee. The fee will be waved for termination or dissolution of a contract with good cause.
